Mechanism of action[edit]

Ciclosporin’s main effect is to lower the activity of T-cells; it does so by calcineurin–phosphatase pathway and preventing the mitochondrial permeability transition pore from opening. Ciclosporin binds to the cytosolic protein cyclophilin (immunophilin) of lymphocytes, especially of T cells. This ciclosporin—cyclophilin complex inhibits calcineurin, which is normally responsible for activating the transcription of interleukin 2. In T-cells, activation of the T-cell receptor normally increases intracellular calcium, which acts via calmodulin to activate calcineurin. Calcineurin then dephosphorylates the transcription factor NF-AT (nuclear factor of activated T-cells), which moves to the T-cell nucleus and increases the transcription of genes for IL-2 and related cytokines.[7] Ciclosporin, by preventing the dephosphorylation of NF-AT, leads to reduced effector T-cell function;[23][24][25][26] it does not affect cytostatic activity.

Ciclosporin also binds to the cyclophilin D protein that constitutes part of the mitochondrial permeability transition pore (MPTP).[24][27] The MPTP is found in the mitochondrial membrane of cardiac muscle cells and moves calcium ions (Ca2+
) into the mitochondria.[24][27] When open, Ca2+
enters the mitochondria and causes the muscle cells (and thus the heart) to contract. If unregulated, the influx of Ca2+
can contribute to mitochondrial swelling and dysfunction.[27]

Pharmacokinetics[edit]

Ciclosporin is a cyclic peptide of 11 amino acids; it contains a single D-amino acid, which is rarely encountered in nature. Unlike most peptides, ciclosporin is not synthesized by ribosomes.[28]

Ciclosporin is highly metabolized in humans and animals after ingestion. The metabolites, which include cyclosporin B, C, D, E, H, and L,[29] have less than 10% of ciclosporin’s immunosuppressant activity and are associated with higher kidney toxicity.[30] Individual ciclosporin metabolites have been isolated and characterized but do not appear to be extensively studied.

Biosynthesis[edit]

Cyclosporin is synthesized by a nonribosomal peptide synthetase, cyclosporin synthetase.[31] The enzyme contains an adenylation domain, a thiolation domain, a condensation domain, and an N-methyltransferase domain. The adenylation domain is responsible for substrate recognition and activation, whereas the thiolation domain covalently binds the adenylated amino acids to phosphopantetheine, and the condensation domain elongates the peptide chain. Cyclosporin synthetase substrates include L-valine, L-leucine, L-alanine, glycine, 2-aminobutyric acid, 4-methylthreonine, and D-alanine, which is the starting amino acid in the biosynthetic process.[32] With the adenylation domain, cyclosporin synthetase generates the acyl-adenylated amino acids, then covalently binds the amino acid to phosphopantetheine through a thioester linkage. Some of the amino acid substrates become N-methylated by S-adenosyl methionine. The cyclization step releases cyclosporin from the enzyme.[33] Amino acids such as D-Ala and butenyl-methyl-L-threonine indicate cyclosporin synthetase requires the action of other enzymes such as a D-alanine racemase. The racemization of L-Ala to D-Ala is pyridoxal phosphate-dependent. The formation of butenyl-methyl-L-threonine is performed by a butenyl-methyl-L-threonine polyketide synthase that uses acetate/malonate as its starting material.[34]

History[edit]

In 1970, new strains of fungi were isolated from soil samples taken from Norway and from Wisconsin in the USA by employees of Sandoz (now Novartis) in Basel, Switzerland. Both strains produced a family of natural products called cyclosporins. Two related components that had antifungal activity were isolated from extracts from these fungi. The Norwegian strain, Tolypocladium inflatum Gams, was later used for the large scale fermentation of ciclosporin.[35]

The immunosuppressive effect of the natural product cyclosporin was discovered in December 1971 in a screening test on immune suppression designed and implemented by Hartmann F. Stähelin at Sandoz.[36][35] The chemical structure of cyclosporin was determined in 1976, also at Sandoz.[37][38] The success of the drug candidate ciclosporin in preventing organ rejection was shown in kidney transplants by R.Y. Calne and colleagues at the University of Cambridge,[39] and in liver transplants performed by Thomas Starzl at the University of Pittsburgh Hospital. The first patient, on 9 March 1980, was a 28-year-old woman.[40] In the United States, the Food and Drug Administration (FDA) approved ciclosporin for clinical use in 1983.[41][42][43][44]

Name[edit]

The natural product was named cyclosporin by the German speaking scientists who first isolated it[35] and cyclosporine when translated into English. Per International Nonproprietary Name (INN) guidelines for drugs,[45] the “y” was replaced with “i” so that the INN for the medication is spelled ciclosporin.

Ciclosporin is the INN and British Approved Name (BAN), while cyclosporine is the United States Adopted Name (USAN) and cyclosporin is a former BAN.

Available forms[edit]

Ciclosporin exhibits very poor solubility in water, and, as a consequence, suspension and emulsion forms of the medication have been developed for oral administration and for injection. Ciclosporin was originally brought to market by Sandoz (now Novartis), under the brand name Sandimmune, which is available as soft gelatin capsules, an oral solution, and a formulation for intravenous administration. These are all nonaqueous compositions.[46] A newer microemulsion,[47] orally-administered formulation, Neoral,[48] is available as a solution and as soft gelatin capsules. The Neoral compositions are designed to form microemulsions in contact with water.

Generic ciclosporin preparations have been marketed under various trade names, including Cicloral (Sandoz/Hexal), Gengraf (Abbott) and Deximune (Dexcel Pharma). Since 2002, a topical emulsion of ciclosporin for treating inflammation caused by keratoconjunctivitis sicca (dry eye syndrome) has been marketed under the trade name Restasis (0.05%). Ikervis is a similar formulation with a concentration of 0.1%.[49] Inhaled ciclosporin formulations are in clinical development, and include a solution in propylene glycol and liposome dispersions.[50][51]

Neuroprotection[edit]

Ciclosporin is currently in a phase II/III (adaptive) clinical study in Europe to determine its ability to ameliorate neuronal cellular damage and reperfusion injury (phase III) in traumatic brain injury. This multi-center study is being organized by NeuroVive Pharma and the European Brain Injury Consortium using NeuroVive’s formulation of ciclosporin called NeuroSTAT (also known by its cardioprotection trade name of CicloMulsion). This formulation uses a lipid emulsion base instead of cremophor and ethanol.[52] NeuroSTAT was recently compared to Sandimmune in a phase I study and found to be bioequivalent. In this study, NeuroSTAT did not exhibit the anaphylactic and hypersensitivity reactions found in cremophor- and ethanol-based products.[53]

Ciclosporin has been investigated as a possible neuroprotective agent in conditions such as traumatic brain injury, and has been shown in animal experiments to reduce brain damage associated with injury.[54] Ciclosporin blocks the formation of the mitochondrial permeability transition pore, which has been found to cause much of the damage associated with head injury and neurodegenerative diseases. Ciclosporin’s neuroprotective properties were first discovered in the early 1990s when two researchers (Eskil Elmér and Hiroyuki Uchino) were conducting experiments in cell transplantation. An unintended finding was that CsA was strongly neuroprotective when it crossed the blood–brain barrier.[55] This same process of mitochondrial destruction through the opening of the MPT pore is implicated in making traumatic brain injuries much worse.[56]

Cardiac disease[edit]

Ciclosporin has been used experimentally to treat cardiac hypertrophy[24][57] (an increase in cell volume).

Inappropriate opening of the mitochondrial permeability transition pore (MPTP) manifests in ischemia[24] (blood flow restriction to tissue) and reperfusion injury[24] (damage occurring after ischemia when blood flow returns to tissue), after myocardial infarction[25] (heart attack) and when mutations in mitochondrial DNA polymerase occur.[24] The heart attempts to compensate for disease state by increasing the intracellular Ca2+
to increase the contractility cycling rates.[27] Constitutively high levels of mitochondrial Ca2+
cause inappropriate MPTP opening leading to a decrease in the cardiac range of function, leading to cardiac hypertrophy as an attempt to compensate for the problem.[27][25]

CsA has been shown to decrease cardiac hypertrophy by affecting cardiac myocytes in many ways. CsA binds to cyclophilin D to block the opening of MPTP, and thus decreases the release of protein cytochrome C, which can cause programmed cell death.[24][27][58] CypD is a protein within the MPTP that acts as a gate; binding by CsA decreases the amount of inappropriate opening of MPTP, which decreases the intramitochondrial Ca2+
.[27] Decreasing intramitochondrial Ca2+
allows for reversal of cardiac hypertrophy caused in the original cardiac response.[27] Decreasing the release of cytochrome C caused decreased cell death during injury and disease.[24] CsA also inhibits the phosphatase calcineurin pathway (14).[24][25][59] Inhibition of this pathway has been shown to decrease myocardial hypertrophy.[25][57][59]

Veterinary use[edit]

The medication is approved in the United States for the treatment of atopic dermatitis in dogs.[60] Unlike the human form of the medication, the lower doses used in dogs mean the drug acts as an immunomodulator and has fewer side effects than in humans.     Cetirizine

    Cetirizine, prominently marketed under the brand name Zyrtec among others, is a potent second-generation antihistamine used in the treatment of hay fever, allergies, angioedema, and urticaria.[5] It acts as a selective antagonist of the histamine H1 receptor.

    Second-generation antihistamines like cetirizine are less able to cross the blood–brain barrier and therefore have diminished effects on the central nervous system compared to first-generation drugs. As such, they are less likely to cause drowsiness or memory impairment.

    Medical uses[edit]

    Allergies[edit]

    Cetirizine’s primary indication is for hay fever and other allergies. Because the symptoms of itching and redness in these conditions are caused by histamine acting on the H1 receptor, blocking those receptors temporarily relieves those symptoms.

    Cetirizine is also commonly prescribed to treat acute and (in particular cases) chronic urticaria, more efficiently than any other second-generation antihistamine[citation needed].

    Rhinovirus infection[edit]

    Interleukin 6 and interleukin 8 have been shown to be elevated in acute respiratory distress syndrome.[6] One recent study of airway epithelial cells showed that levocetirizine, the active enantiomer of cetirizine, may have beneficial effects on the pathophysiologic changes related to human rhinovirus (HRV) infection.[7]

    Kimura’s disease[edit]

    Cetirizine is an effective agent in treating the symptoms of Kimura’s disease which predominantly affects the lymph nodes and soft tissue of the head and neck in the form of tumor-like lesions. Cetirizine’s properties of being effective both in the treatment of pruritus (itching) and as an anti-inflammatory agent make it suitable for the treatment of the pruritus associated with these lesions.[8] In a 2005 study, the American College of Rheumatology conducted treatments initially using prednisone, followed by steroid dosages and azathioprine, omeprazole, and calcium and vitamin D supplements over the course of two years.[8] The skin condition of the patient began to improve and the skin lesions lessened. However, there were symptoms of cushingoid and hirsutism observed before the patient was removed from the courses of steroids and placed on 10 mg/day of cetirizine to prevent skin lesions;[8] an agent suitable for the treatment of pruritus associated with such lesions.[8] Asymptomatically, the patient’s skin lesions disappeared after treatment with cetirizine, blood eosinophil counts became normal,[8] corticosteroid effects were resolved,[8] and a remission began within a period of two months.[8] It is also thought that the inhibition of eosinophils may be the key to treatment of Kimura’s disease due to the role of eosinophils, rather than other cells with regards to the lesions of the skin.[8]

    Available forms[edit]

    Cetirizine is available over-the-counter in the form of 5 and 10 mg tablets. A 20mg strength is available by prescription only.[1]

    Adverse effects[edit]

    Commonly reported side effects of cetirizine include headache (16%), dry mouth (5.7%), drowsiness (5–20%), and fatigue (5.6%), while more serious but rare side effects include cardiac failure, tachycardia, and edema.[9]

    Withdrawal[edit]

    A 2016 study published in Drug Safety suggests “a causal relation between withdrawal of (levo)cetirizine and the occurrence of unbearable pruritus”, concluding that “healthcare professionals should be aware that in some patients this reaction can occur.” Patients studied were found to have been using cetirizine continuously for months or years.[10]

    Long-term daily usage of cetirizine may result in what resembles antihistamine dependency. No official study has been conducted to determine how long a patient may take cetirizine daily before expecting to encounter withdrawal symptoms when treatment stops. Many patients seeking help with withdrawal symptoms report having taken cetirizine for more than three years, while others report having taken it for no longer than one week.[11][12]

    Insatiable, generalized itching is the most commonly-reported withdrawal side effect, though others have reported severe rebound of original, pre-treatment systems as well, including sneezing and runny nose.[13][14]

    Pharmacology[edit]

    Pharmacodynamics[edit]

    Cetirizine acts as a highly selective antagonist of the histamine H1 receptor.[1] The Ki values for the H1 receptor are approximately 6 nM for cetirizine, 3 nM for levocetirizine, and 100 nM for dextrocetirizine, indicating that the levorotatory enantiomer is the main active form.[1] Cetirizine has 600-fold or greater selectivity for the H1 receptor over a wide variety of other sites, including muscarinic acetylcholine, serotonin, dopamine, and α-adrenergic receptors, among many others.[1] The drug shows 20,000-fold or greater selectivity for the H1 receptor over the five muscarinic acetylcholine receptors, and hence does not exhibit anticholinergic effects.[15][16] It shows negligible inhibition of the hERG channel (IC50 > 30 µM)[17] and no cardiotoxicity has been observed with cetirizine at doses of up to 60 mg/day, six times the normal recommended dose[1] and the highest dose of cetirizine that has been studied in healthy subjects.[18]

    Cetirizine crosses the blood–brain barrier only slightly, and for this reason, it produces minimal sedation compared to many other antihistamines.[19] A positron emission tomography (PET) study found that brain occupancy of the H1 receptor was 12.6% for 10 mg cetirizine, 25.2% for 20 mg cetirizine, and 67.6% for 30 mg hydroxyzine.[20] (A 10 mg dose of cetirizine equals about a 30 mg dose of hydroxyzine in terms of peripheral antihistamine effect.)[21] PET studies with antihistamines have found that brain H1 receptor occupancy of more than 50% is associated with a high prevalence of somnolence and cognitive decline, whereas brain H1 receptor occupancy of less than 20% is considered to be non-sedative.[22] In accordance, H1 receptor occupancy correlated well with subjective sleepiness for 30 mg hydroxyzine but there was no correlation for 10 or 20 mg cetirizine.[20] As such, brain penetration and brain H1 receptor occupancy by cetirizine are dose-dependent, and in accordance, while cetirizine at doses of 5 to 10 mg have been reported to be non-sedating or mildly sedating, a higher dose of 20 mg has been found to induce significant drowsiness in other studies.[20]

    Cetirizine has been shown to inhibit eosinophil chemotaxis and LTB4 release.[23] At a dosage of 20 mg, Boone et al. found that it inhibited the expression of VCAM-1 in patients with atopic dermatitis.[23]

    Pharmacokinetics[edit]

    Absorption[edit]

    Cetirizine is rapidly and extensively absorbed upon oral administration in tablet or syrup form.[1] The oral bioavailability of cetirizine is at least 70% and of levocetirizine is at least 85%.[3] The Tmax of cetirizine is approximately 1.0 hour regardless of formulation and its onset of action has been reported to be as early as 20 minutes.[2] The pharmacokinetics of cetirizine have been found to increase linearly with dose across a range of 5 to 60 mg.[1] Its Cmax following a single dose has been found to be 257 ng/mL for 10 mg and 580 ng/mL for 20 mg.[2] Food has no effect on the bioavailability of cetirizine but has been found to delay the Tmax by 1.7 hours (i.e., to approximately 2.7 hours) and to decrease the Cmax by 23%.[1][2][24] Similar findings were reported for levocetirizine, which had its Tmax delayed by 1.25 hours and its Cmax decreased by about 36% when administered with a high-fat meal.[24] Steady-state levels of cetirizine occur within 3 days and there is no accumulation of the drug with chronic administration.[2] Following once-daily administration of 10 mg cetirizine for 10 days, the mean Cmax was 311 ng/mL.[25]

    Distribution[edit]

    The mean plasma protein binding of cetirizine has been found to be 93 to 96% across a range of 25 to 1,000 ng/mL independent of concentration.[2] Plasma protein binding of 88 to 96% has also been reported across multiple studies.[3] The drug is bound to albumin with high affinity, while α1-acid glycoprotein and lipoproteins contribute much less to total plasma protein binding.[3] The unbound or free fraction of levocetirizine has been reported to be 8%.[3] The true volume of distribution of cetirizine is unknown but is estimated to be 0.3 to 0.45 L/kg.[1][3] Cetirizine poorly and slowly crosses the blood–brain barrier, which is due mainly to its chemical properties but also to a minor extent to its activity as a P-glycoprotein substrate.[3]

    Metabolism[edit]

    Cetirizine does not undergo extensive metabolism.[1] It is notably not metabolized by the cytochrome P450 system.[26] Because of this, it does not interact significantly with drugs that inhibit or induce cytochrome P450 enzymes such as theophylline, erythromycin, clarithromycin, cimetidine, or alcohol.[1] While cetirizine does not undergo extensive metabolism or metabolism by the cytochrome P450 enzyme, it does undergo some metabolism by other means, the metabolic pathways of which include oxidation and conjugation.[1][2] Plasma radioactivity attributed to unchanged cetirizine is more than 90% at 2 hours, 80% at 10 hours, and 70% at 24 hours, indicating limited and slow metabolism.[2] The enzymes responsible for transformation of cetirizine have not been identified.[1]

    Elimination[edit]

    Cetirizine is eliminated approximately 70 to 85% in the urine and 10 to 13% in the feces.[1] About 50 or 60% of cetirizine eliminated in the urine is unchanged.[1][2] It is eliminated in the urine via an active transport mechanism.[2] The elimination half-life of cetirizine ranges from 6.5 to 10 hours in healthy adults, with a mean across studies of approximately 8.3 hours.[1][2] Its duration of action is at least 24 hours.[2] The elimination half-life of cetirizine is increased in the elderly (to 12 hours), in hepatic impairment (to 14 hours), and in renal impairment (to 20 hours).[2]

    Chemistry[edit]

    Cetirizine contains L- and D-stereoisomers. Chemically, levocetirizine is the active L-enantiomer of cetirizine. The drug is a member of the diphenylmethylpiperazine group of antihistamines. Analogues include cyclizine and hydroxyzine.

    Synthesis[edit]

    The 1-[(4-chlorophenylmethyl]-piperazine is alkylated with methyl (2-chloroethoxy)-acetate in the presence of sodium carbonate and xylene solvent to produce the Sn2 substitution product in 28% yield. Saponification of the acetate ester is done by refluxing with potash in absolute ethanol to afford a 56% yield of the potassium salt intermediate. This is then hydrolyzed with aqueous HCl and extracted to give an 81% yield of the carboxylic acid product.

    Availability[edit]

    Formerly prescription-only in many countries, cetirizine is now available without prescription in most countries. In some countries it is available over-the-counter only in packages containing seven or ten 10 mg doses.

    Like many other antihistamine medications, cetirizine is commonly prescribed in combination with pseudoephedrine, a decongestant. These combinations are often marketed using the same brand name as the cetirizine with a “-D” suffix (Zyrtec-D, Virlix-D, etc.)
